Hackney's Reablement at Home service supports adults to regain independence and achieve therapy-led goals in their own homes. The service takes a reablement-first approach, focusing on prevention, recovery and reducing reliance on long-term care through short-term, structured interventions.

As an Occupational Therapist within this service, you will play a pivotal role in delivering strengths-based, person-centred reablement, helping residents rebuild skills, confidence and control over their daily lives.

Working closely with reablement providers, Senior Practitioners and Occupational Therapy Assistants, you will:

  • Complete comprehensive OT assessments within the community
  • Develop and issue Reablement Therapy Plans at the point of referral
  • Support improved flow through hospital discharge pathways
  • Ensure support is risk assessed, proportionate and outcome focused

You will be an experienced Occupational Therapist with a strong interest in reablement, prevention and community-based practice.

Requirements:

  • A UK-recognised degree in Occupational Therapy
  • Experience working in community or reablement settings
  • A role embedded in preventative adult social care practice
  • Opportunities to work collaboratively across health and social care
