Hackney Council is seeking an experienced Leaving Care Social Worker to join the Leaving Care Team on a 12-month contract.

You will manage a caseload of care-experienced young people aged 16-25, develop pathway plans, and work with families and partner agencies to achieve positive outcomes.

You will be joining a service committed to relationship-based and anti-racist practice, with strong support and supervision within a vibrant London borough.
