Hackney Council is seeking an experienced Hospital Discharge Social Worker to support adults with complex needs in achieving safe, timely, and sustainable discharges from hospital back into the community.
You will work with hospital wards, community mental health teams, housing services and carers to coordinate discharge planning from admission through to community reintegration, delivering person-centred care.
