About the Role

As a Senior Practitioner within Adult Services, you will play a pivotal role in leading practice, supervising staff, and ensuring safeguarding responsibilities are undertaken to the highest standard. • Support the implementation of self‑directed support, personalisation, and strengths‑based practice.
  • Manage and analyse casework information, ensuring high standards of recording and compliance.
  • Work in partnership with health colleagues, voluntary sector partners, and internal teams to deliver integrated, person‑centred services.
  • Chair case conferences, safeguarding meetings, and multi‑disciplinary forums where required.
  • Contribute to quality assurance frameworks, service development, and continuous improvement initiatives.
  • Promote equality, inclusion, and best practice across the service.

About You

We’re looking for someone who brings:

  • Technical Knowledge & Experience: Post‑qualifying experience working with adults with long‑term conditions, complex needs, or safeguarding concerns. Strong knowledge of the Care Act, Mental Capacity Act, DoLS, and London Multi‑Agency Safeguarding Policy. Ability to support and guide staff through complex assessments and risk‑based decision making. Experience supervising staff and managing performance. Confidence in chairing meetings, writing reports, and contributing to strategic work.
  • Skills & Competencies: Excellent communication, IT, organisational and analytical skills. Ability to work collaboratively as part of a multi‑disciplinary, Think‑Family‑oriented team. Strong legal literacy and ability to represent the service professionally. Commitment to anti‑discriminatory practice and Hackney's equality values. Ability to work flexibly and respond to emerging priorities.
  • Qualifications: Social Work qualification (DipSW, CQSW, Degree). Current Social Work England registration. Enhanced DBS.
