About the RoleAs an Advanced Nurse Practitioner you will manage a broad patient caseload across Urgent Treatment Centres and Out of Hours settings, providing autonomous assessment, diagnosis, treatment and ongoing management of patients presenting with undifferentiated and complex health needs. You will work closely with multidisciplinary teams to ensure high standards of care, clinical safety and seamless patient pathways.
Key Responsibilities- Conduct comprehensive clinical assessments, diagnosis, planning, treatment and review of patients both face‐to‐face and via telephone within urgent care settings.
- Diagnose and manage patients presenting with acute and chronic conditions, including development of appropriate management plans.
- Identify clinical priorities and make timely, safe decisions to support patients presenting with urgent or complex needs.
- Manage patients with diverse physiological and psychological requirements, including those with complex needs, multiple diagnoses and polypharmacy.
- Work collaboratively with clinical teams to deliver high‐quality care and effective patient outcomes.
- Maintain accurate, contemporaneous clinical records and adhere to governance, clinical policies and best practice standards.
- Registered Nurse (NMC) with excellent standing and current UK registration.
- Proven experience in autonomous urgent care, primary care or Out of Hours clinical practice.
- Full MSc in Advanced Nurse Practice or equivalent advanced practice credentials.
- Strong clinical decision‐making and case management skills.
- Experience managing undifferentiated presentations and complex patient needs.
- Excellent communication and multidisciplinary teamwork skills.
