About the Role
As a Care Navigator, you’ll be the first point of contact for patients, carers, and healthcare professionals - providing compassionate, efficient, and professional support over the phone.
Key Responsibilities
- Handle incoming calls for GP practices, ensuring patients are directed to the most appropriate clinician or service.
- Escalate emergency calls promptly to ambulance services.
- Support the wider primary care team with follow-ups and coordination.
- Accurately record patient information using the EMIS Web system.
- Liaise with external services and follow clinical instructions.
- Maintain confidentiality and uphold a patient-first approach.
- Navigate local referral pathways to ensure holistic care.
