About the Role

As a Care Navigator, you’ll be the first point of contact for patients, carers, and healthcare professionals - providing compassionate, efficient, and professional support over the phone.

Key Responsibilities

  • Handle incoming calls for GP practices, ensuring patients are directed to the most appropriate clinician or service.
  • Escalate emergency calls promptly to ambulance services.
  • Support the wider primary care team with follow-ups and coordination.
  • Accurately record patient information using the EMIS Web system.
  • Liaise with external services and follow clinical instructions.
  • Maintain confidentiality and uphold a patient-first approach.
  • Navigate local referral pathways to ensure holistic care.
