We are working with a well-established local authority in North Wales that is seeking an experienced Social Worker to join its dedicated disabilities service. About the Service
You will join a dedicated Disabilities Team supporting children, young people and young adults aged 0-25 with a range of complex needs. The service is committed to child- and young person-centred practice, positive outcomes and close partnership working with families, education, health and other agencies. The team places a strong emphasis on ensuring that every child or young person has access to the tailored support, resources and care they need to thrive.
Key Responsibilities
- Manage a caseload of children, young people and young adults aged 0-25 with disabilities and complex needs
- Complete Child and Family Assessments, statutory assessments and reviews
- Undertake Care Act, transition and Short Break assessments where appropriate
- Develop, implement and review person-centred care and support plans
- Work closely with families, schools, health professionals and multi-agency partners
- Identify, assess and manage safeguarding concerns in line with statutory guidance
- Support Child in Need, Child Protection and Looked After Children processes where required
- Contribute to transition planning for young people moving into adulthood
- Prepare clear, high-quality assessments, reports and court documentation when necessary
- Ensure the views, wishes and feelings of children and young people remain central to all planning and decision-making
About You
- Qualified Social Worker
- Registered with Social Work England
- Recent experience in a Children with Disabilities, 0-25 Disabilities, Safeguarding, Child in Need, Assessment or Transition Team
- Strong knowledge of safeguarding legislation and practice relating to children and young people with disabilities
- Experience supporting children and young people with complex needs
- Confident completing assessments, care plans and safeguarding work
- Skilled in relationship-based practice and multi-agency partnership working
