Job Summary

We have a new and exciting opportunity to appoint an Advanced Nurse Practitioner to join our dynamic and friendly operations team.

The post-holder will have the opportunity to work in our Same Day Urgent Care service.

You will be dealing with a wide range of patient health needs across the local area.

You will be supported in your role through the clinical leads across the services you are working in as well as an experienced and friendly management and administration team.

This post may require you to travel between sites within the borough of Hillingdon as and when required you must be registered with the Nursing and Midwifery Council (NMC) and hold pin and a certificate of completion of training.

  • Primary Responsibilities
  • In accordance with the services needs the post-holder will make him/her-self available to undertake a variety of advanced nurse practitioner duties including prescribing medication, medical cervical smear tests, post-operative dressings, HRT reviews, Pill Checks, Immunisations, BP checks, asthma checks and diabetic reviews and spirometry if trained.
  • Assessing the health needs of patients with chronic health needs and using your own judgment to assess whether further input is needed.
  • Screening patients for disease risk factors and early signs of illness.
  • Contributing to QOF requirements and recording data appropriately.
  • Contributing to PCN requirements and recording data appropriately.
  • In consultation with patients and in line with current practice disease management protocols, developing care plans for health related illnesses.
  • Screening patients for health education.
  • Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.
  • Recording clear and contemporaneous consultation notes to agreed standards.
  • Collecting data for audit purposes and contributing to the practice audit programme as required.
  • Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date.
  • In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a practice nurse working within primary care, as set by Royal College Nursing and the Nursing and Midwifery Council and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.

  • Foster and maintain strong links with all services across the PCN and neighbouring networks.
  • Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.
  • Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e. g.

CCGs).

  • Can recognise personal limitations and refer to more appropriate colleague(s) when necessary.
  • Liaises with other stakeholders as needed for the collective benefit of patients.

Professional Development

  • Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
  • Involved in one to one meetings with line manager monthly to discuss targets and outcomes achieved.
  • Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
  • Review yearly progress and develop clear plans to achieve results within priorities set by others.

  • Recognises priorities when problem-solving and identifies deviations from normal pattern and can refer to seniors when appropriate.

  • The post-holder is required to travel independently between sites (where applicable), and to attend meetings etc. hosted by other agencies.

Person Specification
