The predicted salary is between 46000 - 52000 £ per year.

You are an experienced line manager, ideally from the healthcare sector. You understand the daily operational challenges of service delivery and you are an excellent problem solver. While reactiveness in such a fast-paced environment is vital, you are also adept in long-term planning, process design, and service implementation.

You have experience in employing project management methodologies leading to successful project delivery, monitoring, and evaluation of outcomes. Leading by example, you support your direct reports and peers to ensure excellent patient outcomes. You have a can-do attitude, whilst being acutely aware of your own limitations, knowing when to ask for help and support. Crucially, you have a desire to learn and continue to develop yourself, your direct reports, and the services we deliver. You will bring with you a network of local neighbourhood partners or be adept at making these links swiftly and with confidence.

Main duties of the job:

  • Strong understanding of the NHS including its history, current landscape, and future direction with desirable knowledge of the Wakefield health and social care system and key partners.
  • Leads project management and delivery of proactive integrated neighbourhood models across the PCN aligned to system priorities and outcomes.
  • Establishes governance including planning, risk management, monitoring, and reporting to leadership and the board.
  • Engages people with lived experience to inform service design and improvement and ensures patient safety is central to all work.
  • Leads transformation projects such as the Community Frailty Team and represents the PCN at system meetings to build relationships and influence neighbourhood care.
  • Maintains accurate confidential records including minutes, action, and decision logs.
  • Works with multidisciplinary teams and ARRS staff to design, improve, and integrate high-quality services.
  • Provides line management supervision and development to support effective delivery.
  • Communicates clearly to ensure compliance with standards and legislation and supports staff to follow local policies.
  • Promotes a culture of speaking up and maintains high standards of confidentiality and integrity.
  • Demonstrates strong IT skills across Microsoft 365, AI-supported administration, SystmOne, and other systems with experience using digital tools to enhance and monitor services.

  • Experience within NHS Primary Care, GP Federations, or Community Interest Companies.
  • Experience in planning, designing, and delivering programmes of work or projects.
