An exciting opportunity has arisen at Wakefield North Primary Care Network (PCN) for a Nursing Associate to join our friendly team of Nurses and Nurse Associates working with our Housebound patients and those with a long-term condition living in a nursing or residential home.
Working alongside an experienced Long Term Conditions Nurse, fellow Nursing Associate, Care Co-ordinators, Clinical Pharmacists and GP colleagues, you will help ensure that housebound patients and care home residents receive the same high standard of care as those who are able to attend their GP practice. You will play a key role in improving health outcomes, promoting independence and reducing health inequalities across our local communities.
Main duties of the job
As a Nursing Associate, you will work as part of a multi-disciplinary team to support the delivery of holistic, person-centred care for housebound patients and care home residents living with long-term conditions. You will undertake a range of clinical activities under the direction of registered clinicians, including monitoring and recording clinical observations, supporting long-term condition reviews, carrying out frailty assessments, undertaking NHS Health Checks and promoting health and wellbeing.
The role involves working closely with GPs, nurses, pharmacists, care co-ordinators and other healthcare professionals to ensure patients receive timely, coordinated care. You will support chronic disease management, contribute to preventative healthcare initiatives and help patients, families and carers to better understand and manage their health conditions. Following appropriate training and competency assessment, you may also undertake additional clinical duties such as venepuncture, vaccinations and other delegated procedures within the scope of the Nursing Associate role.
You will maintain accurate clinical records, communicate effectively with patients and colleagues, contribute to quality improvement initiatives and support the ongoing development of services across the Primary Care Network. The role offers the opportunity to make a meaningful difference to patient care while continuing your own professional development within a collaborative and forward-thinking team.

Wakefield North PCN has six GP practices in the Eastern side of the Wakefield District serving a population of around 48,700 and we are looking to recruit an enthusiastic, forward-thinking Nursing Associate to work as part of our established team looking after the health and wellbeing of our local population with complex health conditions.

- Portfolio of learning in relation to knowledge and experience acquired whilst completing their Nursing Associate training
