We are seeking an enthusiastic and compassionate Practice Nurse to join our friendly, forward‑thinking primary care team. The main purpose of the Practice Nurse post is to deliver high‑quality, patient‑centred nursing care within a primary care setting, supporting the health and wellbeing of the practice population. The Practice Nurse contributes to improving patient outcomes, maintaining clinical standards, and ensuring safe, effective, and compassionate care in line with professional and regulatory guidelines.
Key Responsibilities:- Chronic disease management (including diabetes)
