Wakefield North Primary Care Network is seeking a compassionate and motivated Mental Health Practitioner to join our established multi‑disciplinary team supporting patients across six GP practices. The role involves carrying out mental health assessments and providing high‑quality care offering advice, support and signposting to patients experiencing poor mental health.

Working collaboratively with the GPs and the wider primary care team, you will play a vital role in strengthening community resilience and delivering coordinated care across our six member practices. You will be required to provide tailored advice and guidance within your area of expertise, always guided by external regulations, professional standards and legal processes.

Working directly with patients, their carers and families, the role holder will be responsible for identifying requirements of the patient, understanding risk factors and anticipating potential issues which may arise.

  • Provide compassionate, patient-centered mental health support to individuals experiencing poor mental health within GP practices across the PCN.
  • Undertake mental health assessments, risk assessments, and develop personalised care and intervention plans in partnership with patients and carers.
  • Deliver advice, support, signposting, health education, and recovery‑focused interventions through telephone and face-to-face consultations.
  • Monitor patient progress, review care plans, and respond appropriately to changes in mental health needs or crisis situations.
  • Maintain accurate and confidential patient records in line with professional standards, GDPR, and organisational policies.
  • Promote equality, dignity, social inclusion, and positive mental health awareness while helping to reduce stigma associated with mental health conditions.
  • Participate in supervision, training, team meetings, and continued professional development to maintain high standards of clinical practice.

The post holder should have significant experience working within mental health services and be confident working independently and as part of a multi‑disciplinary team. Excellent communication, assessment, care planning and risk management skills are essential, alongside a commitment to delivering compassionate, evidence‑based care.
