We are looking to expand our established Pharmacy team by recruiting a Clinical Pharmacist to work alongside our practices and PCN staff. Our PCN staff team currently include clinical pharmacists, pharmacy technicians, social prescribers, health and wellbeing coaches, care co-ordinators, nurse associates, occupational therapists, mental health nurses, and first contact physiotherapists.
You will need to be committed to improving health and putting patients first. Your ability to build effective relationships with patients, GPs, Nurses, Community pharmacists, Hospital and practice staff will aid you in this role, alongside your excellent communication skills. The role may involve occasional travel to 5 local practices and occasionally further afield.

Main duties of the job:You will be employed as a pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice. You will be supported by a senior clinical pharmacist and GP who will develop and mentor. You will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
In the role you will need to provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation.
