This is a busy, varied and evolving role, with day‑to‑day responsibilities changing in response to the needs and priorities of the individual practices and wider PCN. The role involves a combination of direct patient contact, home and care home visits, assessments, care coordination and supporting clinicians and other members of the multidisciplinary team.

Care Coordinators play an active role in supporting patients with a range of health and care needs, including those with learning disabilities, dementia, frailty and other complex needs. This includes contributing to Learning Disability annual health reviews, dementia reviews and frailty assessments, neighbourhood work, gathering information directly from patients and carers, identifying concerns and ensuring appropriate follow‑up.

The role involves working closely with clinicians, practice teams and external services to coordinate patient care, complete delegated tasks and refer or signpost patients to appropriate support, including voluntary, community and social enterprise (VCSE) organisations. Care Coordinators are also responsible for recognising and escalating concerns, including safeguarding concerns, in line with relevant policies and procedures.

Main duties of the job include managing a varied workload, responding to changing service demands and contributing to the effective delivery of PCN services, helping to ensure patients receive the right care and support at the right time. Appropriate training, supervision and support will be provided to enable the successful candidate to carry out their responsibilities safely and effectively.

Our vision is that all our patients should have the same chance at a happy, healthy life. We believe that a sustainable, resilient Primary Care Service working collaboratively with volunteer and community services and social enterprise organisations supports this vision.
