The Role
We are seeking an experienced and dedicated Care Coordinator to join our Gregory Hills Head Office team on a full-time basis (Monday to Friday).
In this role, you will:

Coordinate and oversee care plans, risk assessments, and service agreements.

Supervise, mentor and support a team of Support Workers.

Conduct client onboarding, home visits, and quarterly check-ins.

Collaborate with clients, families, and stakeholders to ensure person-centered outcomes.

Manage client documentation, incidents, complaints, and continuous improvement processes.

What We’re Looking For
Minimum 2 years’ experience in care coordination or case management
Strong knowledge of NDIS, disabilities, complex care and mental health.
Proven leadership and staff supervision skills.
Excellent communication, organisation, and time management skills.
Current Working with Children Check, NDIS Worker Screening, Police Check, Driver’s Licence, and access to own vehicle.
Relevant tertiary qualification in Disability, Allied Health, Social Science or similar (highly regarded).
