Job Description
The RN Case Manager is responsible for coordinating patient care and ensuring that patients receive comprehensive support throughout their treatment journey.
This role involves collaborating with healthcare teams, patients, and their families to develop and implement individualized treatment plans that address physical, financial, and other constraints.

- Conduct intake assessments with patients and their families to determine needs and care plans.
- Ensure families feel equipped and supported throughout the care process.
- Develop individualized treatment plans that consider physical, financial, and logistical constraints.
- Adjust treatment protocols as necessary based on patient progress and needs.
- Coordinate appropriate follow-up services to promote overall wellness and recovery.
- Facilitate transportation arrangements for patients to and from healthcare facilities.
- Collaborate with healthcare team members to discuss patient progress and optimize care strategies.
- Evaluate patients’ insurance coverage and financial resources to ensure adequate support for treatment.
- Devise innovative solutions or referrals for patients facing financial barriers to accessing care.
Education
- Minimum of an Associate Degree in Nursing.
- RN licensure required; BSN preferred.
- A diploma is acceptable only in states where licensure requirements permit.
- All other licensed or certified healthcare professionals must possess a minimum of a bachelor’s degree; a graduate degree is preferred.
Experience
- Minimum of 2 years of rehabilitation experience preferred.
- Strong Case Management experience required.
License or Certification
- Must be qualified to independently complete assessments within the scope of practice for disciplines such as RN, SW, OT, PT, ST, or Rehabilitation Counseling.
- If applicable, must hold an active license as required by state regulations.
- Must meet eligibility requirements for CCM or ACM certification upon entry into this position, or obtain within two years.
