We are looking for a Palliative Care Co‑Ordinator.
Can you, once trained, provide advice and support for patients and their families or carers? Our care co‑ordinators will help patients navigate health and social care services and signpost to partner and voluntary organisations.
Main duties of the job
Work out of our care co‑ordination centre, manning and triaging phone calls and web enquires. Act as a single point of access for healthcare professionals and patients and families. To signpost and provide information on services as required.
