About the Role

We are looking for a motivated and compassionate Support Worker to join our accommodation-based service in Berkshire. This service supports individuals who are either transitioning from hospital into the community or at risk of hospital admission, offering a safe environment where their needs can be assessed and supported. The service plays a vital role in helping people with complex health and social needs stabilise, recover, and move towards independent living.

Key Responsibilities

  • Provide high-quality, person-centred support to residents with complex needs
  • Carry out needs assessments and develop tailored support and move-on plans
  • Manage a caseload of clients, ensuring regular reviews and progress tracking
  • Work collaboratively with healthcare professionals, housing providers, and other agencies to deliver holistic support
  • Promote independence, wellbeing, and positive health outcomes for residents
  • Maintain accurate records and contribute to a safe, supportive living environment

About You

  • Experience working with individuals with complex or multiple needs (e.g. homelessness, mental health, physical health)
  • Strong communication and interpersonal skills
  • Ability to work both independently and as part of a small, dynamic team
  • A proactive and flexible approach to problem-solving
  • Understanding of safeguarding and professional boundaries
