Support Worker – Accommodation-Based Health & Recovery Service
Location: Southampton
Hours: Full time and part time hours available
Pay: £14.75-£15.50 per hour – temporary and permanent job opportunities
About the Role
We are looking for a motivated and compassionate Support Worker to join our accommodation-based service in Hampshire. This service supports individuals who are either transitioning from hospital into the community or at risk of hospital admission, offering a safe environment where their needs can be assessed and supported.
The service plays a vital role in helping people with complex health and social needs stabilise, recover, and move towards independent living.
Key Responsibilities
- Provide high-quality, person‑centred support to residents with complex needs
- Carry out needs assessments and develop tailored support and move‑on plans
- Manage a caseload of clients, ensuring regular reviews and progress tracking
- Work collaboratively with healthcare professionals, housing providers, and other agencies to deliver holistic support
- Promote independence, wellbeing, and positive health outcomes for residents
- Maintain accurate records and contribute to a safe, supportive living environment
About You
- Experience working with individuals with complex or multiple needs (e.g. homelessness, mental health, physical health)
- Strong communication and interpersonal skills
- Ability to work both independently and as part of a small, dynamic team
- A proactive and flexible approach to problem‑solving
- Understanding of safeguarding and professional boundaries
