We’re looking for a compassionate and proactive Social Prescriber to join our dynamic team. In this vital role, you’ll empower individuals to enhance their health and wellbeing by connecting them with non-clinical community services, activities, and support networks tailored to their needs. Working closely with primary care professionals, voluntary organisations, and local partners, you’ll help tackle key issues such as isolation, mental health, financial worries, housing challenges, and lifestyle management. Key Responsibilities

  • Receive and manage referrals from hospital settings, GPs, nurses, and other professionals, strengthening referral pathways.
  • Work with patients and their families to identify social, emotional, and practical needs, creating personalised, person-centred support plans.
  • Provide information and advice on a wide range of services to promote health, wellbeing, and independence within the community.
  • Support patients to make positive lifestyle changes, helping to prevent ill-health and reduce hospital readmissions.
  • Forge strong relationships with local VCSE organisations, community groups, charities, public services, and the wider health sector.
  • Contribute to reducing demand on statutory services, lowering hospital admissions, and avoiding unnecessary GP appointments.
  • Offer ongoing support, encouragement, and follow‑up to ensure individuals engage with and benefit from services.
  • Maintain accurate records and report on outcomes using appropriate data systems.
  • Take part in team meetings, training, and continuous professional development.
  • Champion a holistic, person-centred approach to health and wellbeing.

Required Skills

  • A passion for helping people to self‑care and self‑manage their health and wellbeing, whilst having our Purpose & Values at the heart of what you do.
  • An ability to build strong relationships and work collaboratively with community leaders, local authority, and professionals.
  • Excellent communication (written, verbal and listening) skills with an ability to contribute to written reports and papers, prioritise tasks and manage time effectively.
  • Competent and secure use of IT, including Email, Internet, MS Office 365 and databases.
  • Great interpersonal skills to work with diverse communities showing empathy and emotional resilience.
  • Flexibility with working hours and location; this is an outreach role and you may be based in the hospital and other partner locations.
  • A “can do” attitude and a high‑quality customer focus, with the ability to multi‑task within a dynamic, fast‑paced, and changing environment.
  • Clean DBS record, which will be secured upon appointment to role.

Qualifications

  • Diploma Level 2 in Health and Social Care, or equivalent qualification or experience.
  • Experience in health promotion, community development, social work, or related fields, with an understanding of the wider determinants of health.
  • Skill in motivational interviewing or coaching techniques, with experience supporting vulnerable or at‑risk populations.

Experience

  • Strong listening and communication skills.
  • Motivated by helping people, with care and empathy.
  • Ability to deliver person‑centred support in a non‑judgmental way.
  • Commitment to collaborative working with all local agencies.
  • Proficient in MS Office, research, and written documentation, with strong ability to prioritise tasks, multitask, and manage workload efficiently in a fast‑paced environment.
  • Experience of working within healthcare, the voluntary or community sectors, supporting vulnerable groups.
  • Familiarity with local resources and services and how to access them.
