As a Carer & Dementia Coach & Community Engagement Worker, you will support adult Carers and people living with dementia and their Carers through periods of change, challenge and adjustment. You will:

  • provide intensive, time‑limited support when Carers need specialist input
  • offer direct support to people living with dementia who access the service independently
  • help people navigate systems, understand options and make informed choices
  • connect individuals to services, activities, peer support and their wider community

Your work will help prevent crisis, reduce isolation and support people to live well with dementia.
