Diversion Case Manager Individual Homelessness Pay Range 2400 to 2700 per hour

Position Summary

The Diversion Case Manager is responsible for providing case management to participants and providing advocacy support and referral information and assisting individuals to overcome barriers to becoming self sufficient

Essential Functions and Responsibilities

  • Conduct outreach and assessments in the community Greater Lowell and Merrimack Valley Region
  • Evaluate and assess referrals using a Housing First framework
  • Collaborate with local homeless service providers to facilitate smooth transitions from shelters to permanent housing
  • Maintain accurate case records and collect data for statistical analysis
  • Develop and implement participant centered service plans focusing on achieving participant goals
  • Monitor participant progress identifying strengths needs barriers and goals to sustain housing and increase self sufficiency
  • Educate participants on daily living activities and refer them to necessary service providers
  • Conduct home visits and assist participants in accessing community resources
  • Uphold the values of CTI Equity Integrity Compassion

Qualifications

  • High School Diploma or equivalent education
  • Relevant experience in case management or a related field
  • Knowledge of community resources for low income individuals and families
  • Empathy and understanding of homelessness and related issues
  • Excellent customer service skills and ability to communicate effectively with supervisors coworkers volunteers donors and participants
  • Ability to manage multiple priorities in a fast paced environment
  • Proficiency in basic computer software and data collection systems
  • Strong crisis management skills
  • Ability to maintain accurate records and confidentiality
