The role of this Case Manager is to provide case management to participants and to provide advocacy, support, and referral information and assist individuals to overcome barriers to become self-sufficient.
Essential Functions and Responsibilities:- Complete intake and needs assessments with each participant, which may involve a broad range of areas including housing, finance & budgeting, employment, education, food & nutrition, school & daycare, legal issues, health, parenting, and daily living skills.
- Interview and evaluate clients to formulate Individualized Service Plans based on Program requirements.
- Work closely with clients to identify and build upon strengths and develop strategies to address barriers and concerns identified through the assessment process.
- Help clients build self-sufficiency plans.
- Assist clients to transition to self-advocacy roles.
- Advocate on behalf of clients, and attend administrative hearings if necessary.
- May perform home visits and/or unit inspections.
- Establish, evaluate and monitor goals for clients linked to measurable outcomes.
- Work collaboratively with interdisciplinary team of collateral providers and partners to ensure coordination of services.
- Maintain the structure and policies of the program through orientation of participants by reviewing program rules, policies, and case management visits.

Qualifications:- High School Diploma or equivalent education.
- Prior related experience.
- Minimum of one year assessment, advocacy and case management work in a Human Service field.
- Knowledge and experience working with low income, culturally diverse, and at-risk families and individuals.
- Demonstrated knowledge of community resources available to low-income individuals and families.
- Excellent customer service, organizational, oral and written communication skills.
- Ability to manage multiple priorities and work in a fast-paced environment.
- Proficient computer skills required in basic computer software and data collection systems.
- Ability to maintain accurate records and confidentiality of information.
- Excellent attention to detail.
- Valid Driverβs License.
