Hours: Monday to Friday, 09:00 - 18:30
Reports to: Coordinator Lead & Clinical Lead, CPA
Location: Rotherham (home visits across PCN areas)
Main duties of the job
- Clinical Duties: Provide same-day home visits for housebound patients across Rotherham PCNs. Assess, diagnose and manage acute presentations. Complete verification of death (training provided). Record all consultations on SystmOne and prioritise visits by clinical need.
- Referral & Communication: Respond to GP referrals via SystmOne. Communicate clearly on prescription tasks, follow-ups and concerns. Ensure patients can be contacted and appointments managed efficiently.
- Governance & Compliance: Maintain accurate clinical records and meet governance, supervision and audit requirements. Support incident or complaint reviews. Keep registration, training and competencies up to date and follow all policies and safeguarding duties.
- Service Standards: Support required daily visit levels. Advise patients that visit times are approximate and may occur up to 6pm. Attend review meetings and contribute to reporting, including utilisation and DNAs.
Essential: Professional registration, ability to work autonomously, strong clinical skills, DBS, right to work, indemnity and two references.
Desirable: Experience with frail patients and SystmOne.

Person Specification
- Qualifications: Registered healthcare professional (e.g., Nurse, Paramedic, etc.) Current professional registration with the relevant governing body (NMC, HCPC, Masters level qualification in Advanced Clinical Practice (or working towards it).
- Experience working in GP or primary care settings. Confident managing acute, undifferentiated presentations. Comfortable working autonomously.
- Registered with NMC, HCPC, or equivalent professional body.
- Proven ability to work autonomously in primary, urgent, or community care.
- Strong assessment, diagnostic, and clinical decision-making skills.
- Enhanced DBS clearance.
- Right to work in the UK.
- Suitable clinical indemnity.
- Two satisfactory references.
- Experience working with frail or housebound patients.
- Experience using SystmOne.
