Location: St James’ University Hospital, Leeds

Salary: ÂŁ26,459 pro rata

Hours: Part Time: 56 hours over the course of a fortnight. This averages at 28 hours per week and requires working alternate weekends. Current service hours are between 08:30am and 7pm but these may alter based on the needs of the service and funding.

ABOUT THE ROLE: In this role you will work as part of the Hospital to Home Team, who provide transportation to facilitate hospital discharge and who carry out support tasks within people’s homes to prevent further hospital admissions. The team also assesses individuals’ wider support needs and effectively signposts and refers onto other organisations to ensure longer term support is put in place.

ABOUT THE HOSPITAL TO HOME SERVICE: Our service prides itself on its “Home First” and patient-centred approach and supports those over 50, who have suffered a period of illness that has affected their independence following a hospital admission or visit to A&E. We work closely with professionals from across the Health & Social Care sector and a range of other agencies to encourage clients to access services that will improve their health and wellbeing and retain their independence. Full training will be provided.

PLEASE NOTE: A full driving license and access to a vehicle for “Business Use” is required.
