Job Description
The Community Connectors programme is a partnership between Community Links, Aston Mansfield, Mind in Tower Hamlets, Newham and Redbridge, and the NHS East London Foundation Trust (ELFT). The Senior Community Connector works alongside Community Connectors, providing supervision, caseload reviews, guidance, and day‑to‑day support, and manages a smaller caseload of people living with serious mental illness (SMI).
The role includes leading collaboration with multi‑disciplinary teams (MDTs) across primary care networks (PCNs) in Newham, developing support pathways, identifying emerging needs, linking local experts and community assets, and acting as an ambassador for the programme to promote its value.
Responsibilities
Provide supervision, caseload reviews, guidance, and day‑to‑day support to Community Connectors.
Manage a small caseload of individuals living with SMI.
Lead development of strong relationships with MDTs across PCNs.
Analyse emerging needs and develop further support pathways for service users.
Act as link between local experts and community assets.
Support the team to meet programme aims and objectives.
Serve as ambassador for the Community Connector team, ensuring professional publicity of the programme’s value.
Qualifications
University degree and/or professional qualification.
Local knowledge of services, provisions, and issues faced by people with mental health problems.
Experience working with stakeholders and partnerships.
Commitment to implementing the mission and values of Community Links.
Other relevant duties as deemed necessary.
Additional Information
Contract: Permanent
Hours & work pattern: Full time, 37 hours per week.
Salary: £35,000
Location: Community Links, Barking Road, London. Formal location within one of the CIMHS teams in Newham.
Screening: Successful admission subject to enhanced DBS check and UK right to work. No visa sponsorship available.
