Applicants for this vacancy need to be Female and hold a full UK Driving Licence. This is a 37.5 Hrs per week position. This is an exciting opportunity to join Community Integrated Care at our supported living service in Hemsworth.

What is important to us? We need empathic, patient staff to provide person-centered support to individuals who are living here. We want everyone who accesses our services to have the opportunity to live the best life possible. You will be supporting people to work towards outcomes, enabling them to maintain independence.

You will encourage and support people to engage in the local community, clubs, cinema, shows, days out, gym, pub lunches shopping, and activities of personal choice, they enjoy swimming, family activities, and day trips. You will support a community within Wakefield itself, across several services there are long-standing friendships, and you will support to maintain these relationships.

Moorshutt is a supported living project for 3 individuals with physical and learning disabilities, 24 hours a day. The property is a bungalow on the corner plot of a moderately busy street. It has a large communal open plan living space, it is wheelchair accessible throughout, a specially adapted bathroom, ensuites, a large, grassed garden and plenty of parking.

Other information: We will have a core background team on duty 24/7 so there are overnight shifts, and we will provide additional 1:1 support, ranging from personal care, housekeeping, and meals to days out and trips to the shops and activities.
