We are looking for Advanced Support Workers that have experience in Autism, Complex Care needs and Learning Disabilities. Community Integrated Care is recruiting for an Advanced Support Worker with flexible shift patterns. Responsibilities
- Provide person-centred support to three young men in a supported living home, enabling them to develop independence and maintain safety in their home and community.
- Support day-to-day activities including personal care, housekeeping, meal preparation and cooking, and participation in local activities.
- Maintain accurate records and medication administration as required.
- Actively engage with families, professionals, and services to coordinate care and support aspirations.
- Ensure all actions align with the principles of choice, dignity and respect.
- Work within shift patterns: day shifts 8:00‑20:00, waking nights 20:00‑08:00, and two 1‑hour blocks supporting community-based activities.
Qualifications and Experience
- Full UK driving licence (any class).
- Experience supporting individuals with autism, learning disabilities, ADHD, epilepsy and complex care needs.
- Experience with care leavers and youth transition services is an advantage.
- Strong interpersonal skills, empathy, patience and the ability to make timely decisions.
- Willingness to work both day and night shifts and 36 hours per week in a 24/7 duty cycle.

- Brackley House is a detached five‑bedroom property in Heaton Chapel, Stockport, housing four young men aged 19‑24 with complex care needs.
- We provide a supportive and engaging environment that encourages life skills and community participation.
- All staff are supported by a team on duty 24/7, with clear shift coverage.
