Community Integrated Care is currently seeking an experienced, dedicated and compassionate Health and Social Care Professional for the Service Leader – Level 1 position in Southport.

Responsibilities
You will be responsible for leading an engaged and motivated team of support workers to deliver person‑centred outcomes for every individual. Your duties will include driving performance, setting the pace and direction of services, promoting a positive and passionate culture, ensuring services are adequately resourced, and maintaining quality of care at all times. You will also manage intensive support plans for people with behaviours requiring heightened safety and quality of life considerations, and will use basic IT systems to record and track care.
Qualifications
With experience in a health or social care environment, you will have a positive, upbeat manner and a genuine passion for supporting people in their local communities. You must hold a professional qualification in Health and Social Care (e.g. registered nurse, social worker, SVQ Level 3 with supervisory units in Scotland, or Care Management Level 3 in England) and be able to lead a team effectively.
