Position Summary: Community Health Action of Staten Island, in partnership with Sun River Health, has developed a collaborative program to enhance care engagement, improve HIV health outcomes, and promote the well-being of people living with HIV/AIDS (PLWH/A) in Queens and Staten Island. The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. This program will expand services to Staten Island and establish community-based, non‑Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs.

Duties & Responsibilities:

  • Complete assessments and develop a coordinated service plan with the client, the client’s family, and the care team.
  • Document the client’s progress towards treatment and monitor client progress.
  • Collaborate with Health Educator and Peer Advocate to identify appropriate health education services for clients.
  • Manage the Healthy Food Security intervention, providing nutrition counseling/education, grocery gift cards and development of food security plan.
  • Maintain updated case records through the agency’s EMR in accordance with Health Research Services Administration (HRSA) defined services, agency standards and regulatory requirements.
  • Accompany clients to medical appointments and community agencies for support and advocacy.
  • Assess treatment adherence and address barriers as they arise.
  • Reassess client needs including the Self‑Management Assessments and program eligibility.
  • Coordinate with Service Providers and provide critical feedback to members of the team.
  • Actively participate in case conferences, clinical supervision, and individual supervision.
  • Meet training requirements as indicated by the funder including the 6‑day Core training.
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
  • Other duties as assigned.

Qualifications:

  • High school diploma or equivalency required.
  • At least 2 years of related experience in substance use disorders, HIV and/or mental health settings required.
  • Strong socio‑cultural identification with the target population.
  • Some college, Associate’s, or bachelor's preferred.
  • Bilingual (English/Spanish) preferred.

Competencies:

  • Computer literacy in Microsoft products: TEAMS, Outlook, etc., and ability to navigate the internet.
  • Knowledge, understanding, and ability to communicate HIV progression, treatments, and adherence strategies.
  • Knowledge and understanding of care management principles and documentation.
  • Can actively listen to feedback and apply that feedback accordingly.
  • Compose documentation, emails, and other communications that are clear, accurate, and well‑organized.
  • Understand and convey information and instructions so that the message accomplishes its intended purpose clearly with colleagues, clients, and external parties.
  • Work harmoniously with others to accomplish objectives, gaining the cooperation and respect of others in the process.
  • Relates effectively with staff, clients, supervisors, and senior management.
  • Show up professionally each day within the context of expectations of their work environment.
  • Work independently following receipt of instructions on a task.

Status: Full‑Time / Non‑Exempt

Pay Rate: $41,600-46,600 annually, commensurate with qualifications; excellent benefits package.
