About Us
Community Case Management is a case management company. We are registered in Wales with Care Inspectorate Wales (CCM Cardiff & Vale) and follow Welsh law and regulations. Case Management supports people who have experienced serious, life‑changing injuries so they can live as independently and fully as possible. At CCM we don't just provide care – we coordinate everything that our client might need to rebuild their life. This includes:
- Health and rehabilitation
- Education and work goals
- Daily living support
- Therapies and equipment
CCM is not a care agency. We guide and manage the care and support teams who are employed directly by the client (via a solicitor or Court of Protection Deputy).
