About Us:
Community Case Management is a case management company. Case Management supports people who have experienced serious, life-changing injuries so they can live as independently and fully as possible. At CCM we don’t just provide care - we coordinate everything that our client might need to rebuild their life. This includes health and rehabilitation, education and work goals, daily living support, therapies and equipment. CCM is not a care agency. We guide and manage the care and support teams who are employed directly by the client (via a solicitor or Court of Protection Deputy). Support workers work with one client, based on the client’s specific needs. CCM provide direction, supervision and training to support workers so they feel confident and capable in their role.
