Join an Established, Trusted Local Care Provider. Community Care Team is a well-established domiciliary care provider based in Leyland. Founded in 1992, we are proud to remain under the same ownership, with a dedicated management team that has been in place for over 20 years. Due to continued growth, we are seeking an experienced Care Coordinator to join our friendly and supportive team.

About the role

This is a full-time, primarily office-based Care Coordinator position within a busy domiciliary care service; however, the role also includes going out into the community to complete assessments, client reviews, staff spot checks, etc. You will support the delivery of safe, effective, and person-centred care in line with CQC standards. Own transport is essential. The role includes participation in the on-call rota (1 weekend in 4 and occasional evenings to provide guidance and support to care staff).

Key responsibilities

  • Completing client assessments and risk assessments using an electronic call monitoring system (Birdie)
  • Creating and updating care plans
  • Managing daily alerts and care notifications on Birdie
  • Planning and maintaining staff rotas using Birdie
  • Conducting regular client reviews
  • Carrying out spot checks and supervisions
  • Completing moving & handling and medication competencies for Community Support Worker staff
  • Managing staff performance and providing ongoing support
  • Answering the telephone and handling day-to-day office enquiries
  • Liaising with service users, families, social workers, and healthcare professionals

Essential Experience and Skills

Applicants MUST have a minimum of 1 year experience in domiciliary care as a Care Coordinator or Team Leader. You will need experience in:

  • Digital care planning and management systems (e.g., Birdie)
  • Completing and reviewing risk assessments
  • Creating and updating care plans
  • Rota planning and staff scheduling
  • Managing and supporting care staff
  • Conducting spot checks and supervisions
  • Liaising with service users, families, social workers, and healthcare professionals
  • On-call
