Community Care Live is seeking a Transitions Social Worker for St Helena Island on a 2-year FTC, with a total package up to £59k per annum. The role leads transition planning for children, young people and care leavers across Children’s and Adult Social Care, ensuring continuity and person-centred plans.
You will undertake high-quality assessments, manage a caseload, collaborate with multi-agency partners, and contribute to safeguarding while promoting independence and positive outcomes.
