Community Care Live is seeking a Senior Case Worker to support adults with care and support needs in achieving smokefree goals.

You will deliver personalised, person-centred quit-smoking support in community settings, collaborating with Social Workers and Public Health colleagues.

You'll assess needs, make referrals to local stop smoking services, and use motivational approaches to overcome barriers to quitting.
