Community Care Live is seeking a Senior Case Worker for our Younger Adults Review Team.

You will manage a diverse caseload, guide colleagues, and model best practices to ensure high standards of social work.

You will review countywide care packages across Physical Disabilities, Learning Disabilities and Mental Health to ensure ongoing suitability, while supporting individuals to maximise wellbeing and independence.
