Qualified Social Worker needed for our Adult Social Care in South Gloucestershire paying £36.75 ph!
About the Role
This role operates on a hybrid working model, with a minimum of 3–4 days per week office- or community-based, depending on service needs and work output. Office attendance and flexibility will be reviewed regularly, and greater flexibility may be offered where work throughput is consistently high and of a good standard. This is an essential Social Work role within Adult Social Care. You will be responsible for carrying out statutory social work assessments, identifying needs, and providing advice and guidance to service users to promote wellbeing, independence, and positive outcomes.
Key Responsibilities
- Undertake needs assessments, reviews, and support planning in line with relevant legislation and best practice.
- Work closely with health professionals, voluntary organizations, and partner agencies as part of a multi-disciplinary team, ensuring effective communication and shared learning.
- Review and monitor care plans and the quality-of-service provision, responding promptly to changes in need.
- Develop, implement, and evaluate reablement programmes within care packages, supporting service users to maximize independence.
- Manage your own caseloads and/or specific projects, ensuring appropriate specialist input and the achievement of effective outcomes.
- Raise awareness of care and safeguarding issues, including their wider implications.
- Complete risk assessments and develop safe methods of work practice guides, ensuring risks are appropriately managed and actions are clearly recorded.
