Location: Fylde & Wyre Area, Lancashire (Lancashire County Council)

Contract: Full-Time / Temporary

About the role:

Lancashire is looking for an experienced Social Worker to join their Adults Acute Services team within the Wyre CAPSS service. This is a fast-paced and rewarding role focused on supporting individuals with complex needs, particularly in hospital discharge settings. You will play a key role in delivering high-quality, strengths-based social work practice, promoting independence and supporting recovery for adults and their carers.

Key Responsibilities:

  • Deliver a professional social work service, fulfilling statutory social care duties
  • Support safe, timely, and effective hospital discharges in collaboration with health colleagues
  • Undertake assessments, care planning, and reviews focused on outcomes and independence
  • Work in partnership with multi-agency professionals to meet complex needs
  • Promote choice, control, and wellbeing for individuals and their carers

Requirements:

  • Qualified Social Worker with a recognised Social Work degree
  • Minimum 3 years’ experience in a permanent role within a Local Authority
