About the role:
Lancashire is looking for an experienced Social Worker to join their Adults Acute Services team within the Wyre CAPSS service. This is a fast-paced and rewarding role focused on supporting individuals with complex needs, particularly in hospital discharge settings. You will play a key role in delivering high-quality, strengths-based social work practice, promoting independence and supporting recovery for adults and their carers.
Key Responsibilities:
- Deliver a professional social work service, fulfilling statutory social care duties
- Support safe, timely, and effective hospital discharges in collaboration with health colleagues
- Undertake assessments, care planning, and reviews focused on outcomes and independence
- Work in partnership with multi-agency professionals to meet complex needs
- Promote choice, control, and wellbeing for individuals and their carers
Requirements:
- Qualified Social Worker with a recognised Social Work degree
- Minimum 3 years’ experience in a permanent role within a Local Authority
