We’re working with a dedicated Older Adults Mental Health team looking for an experienced Social Worker to join their service. What You’ll Be Doing:- Working within an Older Persons Mental Health service supporting older adults with complex mental health needs
- Completing assessments and developing tailored care plans based on individual needs
- Reviewing and adapting packages of care to ensure they continue to meet service users’ needs
- Managing a balanced caseload while delivering safe, effective, and person-centred support
- Working collaboratively with families, carers, healthcare professionals, and wider multidisciplinary teams
- Supporting positive outcomes through strengths-based and recovery-focused practice
- Contributing to the development and support of junior colleagues where required

What We’re Looking For:- Qualified Social Worker
- Current registration with Social Work England (SWE)
- Diploma/Degree in Social Work or a subject related to Mental Health
- Experience working within Older Adults Mental Health, Community Mental Health, or a similar service
- Strong assessment and care planning skills
- Evidence of ongoing professional development
- Full UK driving licence
