We are looking for an Adult's Social Worker to join a Mental Health Discharge Team. Do not apply for this role if you do not have 2 years' of permanent experience post Social Work qualification.

About the team

This team works with individuals aged 18+ in hospital settings ready to be discharged. A key responsibility is to ensure safe, person-centred discharge from hospital by assessing needs and arranging the right support to promote independence and wellbeing. The team will be required to carry out face-to-face assessments, Care Act 2014 and MCA assessments.

About you

It is essential to have a degree within Social Work (Degree/DipSW/CQSW) with a minimum of two years' experience within adult's Social Work post qualified. Experience working in a hospital, CHC or mental health assessment team lends well to the requirements of the role. A valid UK driving licence and regular access to a vehicle is essential in order to be considered for this role.
