About the Role:
We are currently recruiting an experienced Adult Social Worker to join a Community Learning Disability Team on a locum basis. This is an excellent opportunity for a skilled practitioner with 2+ years post-qualifying experience to work in a supportive, multi-disciplinary environment that values person-centred practice and independence. The role focuses on promoting wellbeing, independence, and ensuring appropriate care planning for adults with learning disabilities living in the community.
Key Responsibilities:
- Manage a caseload of adults with learning disabilities
- Complete Care Act assessments and reviews
- Develop and implement person-centred support plans
- Conduct Mental Capacity Act assessments and Best Interest decisions
- Work in partnership with families, providers, health professionals, and advocates
- Safeguarding adults at risk and contributing to investigations
- Maintain accurate and up-to-date case records
