What you’ll be doing:
You’ll be supporting safe hospital discharges, completing and overseeing Care Act assessments, and putting practical care plans in place to help prevent re-admissions. Alongside your own casework, you’ll supervise a small team and work closely with health colleagues to keep discharges moving and make sure people don’t get stuck in hospital unnecessarily.
You’ll need:
- Social work qualification
- Registration with Social Care Wales
- 3+ years post qualifying experience preferred
