The London Borough of Haringey is looking for a proactive and dedicated Social Work Officer to join our Adult Social Care team. In this role, you will work within a strengths-based model of practice, holding a non-complex caseload designed to support residents in improving their independence and wellbeing. You will work closely with partners across the borough, maximizing the use of community resources to help residents lead fulfilling, resilient lives.
Key Responsibilities
- Assessments & Planning: Undertake strengths-based assessments, reviews, and support planning to build resident resilience and improve quality of life.
- Practical Support: Assist residents and carers with accessing living aids and navigating the welfare benefit system to provide vital advice and information.
- Carer Support: Arrange short-term care and respite services to support carers and prevent carer breakdown.
- Collaboration: Liaise with colleagues across the Haringey health and social care partnership to gather information for assessment activities.
- Advocacy: Work with clients, relatives, and communities to help them make informed decisions and express their specific needs.
- Compliance & Reporting: Maintain accurate, timely case notes and support Social Workers in gathering information for legal presentations.
- MDT & Safeguarding: Contribute to Multi-Disciplinary Team (MDT) meetings and attend safeguarding strategy meetings when required.
- Healthcare Integration: Complete Continuing Health Care (CHC) checklists and identify cases requiring full assessments.
Requirements
- Experience working within a social care setting, ideally with adults.
- Strong understanding of the strengths-based approach and the Care Act 2014.
- Excellent communication skills to clarify client needs and contribute to support planning.
- Ability to work effectively within an MDT environment to achieve the best outcomes.
- Proficiency in maintaining reliable digital case records and report writing.
