About The Role
As part of the Neighborhood Support Team, you will:
- Carry out Care Act assessments and reviews within the community
- Support adults with identifying needs, strengths, and appropriate support plans
- Work collaboratively with service users, families, carers, and partner agencies
- Promote independence and person‑centred practice
- Maintain accurate and timely case records and assessments
- Travel across rural areas within Lewes and North Wealden to undertake community visits
Due to the nature of the role, the successful candidate must be a car user.
Key Responsibilities
To be considered for this position, you will:
- Be a qualified and registered Social Worker
- Have at least 2 years’ recent experience working within a similar Adult Social Care or community‑based team
- Have experience completing Care Act assessments and reviews
- Be confident managing community‑based caseloads independently
- Possess strong communication and assessment skills
- Be organized, adaptable, and committed to delivering high‑quality support
