About the Role
We are seeking an experienced and committed Mental Health Social Worker to join the Adult Locality Mental Health Team in Hounslow. This is a great opportunity to work within a supportive multi-disciplinary environment, delivering high‑quality, strengths‑based social care to adults with mental health needs. You will play a key role in supporting service users to maintain independence, manage risk, and access appropriate care and support within the community.
Key Responsibilities
- Manage a caseload of adults with mental health needs within a statutory social work setting.
- Undertake Care Act assessments (2014), safeguarding enquiries, and Mental Capacity Assessments.
- Work collaboratively with health professionals, service users, carers, and partner agencies.
- Support risk management planning and promote recovery‑focused outcomes.
- Maintain accurate and timely case records using relevant systems.
- Contribute to safeguarding processes and multi‑agency decision‑making.
- Work within a flexible and responsive service to meet changing demand.
Requirements
- At least 2 years’ experience working within a Local Authority Mental Health setting.
- Strong knowledge and experience of Care Act 2014 assessments, safeguarding, and Mental Capacity Act work.
- Proficiency in using Liquid Logic case management system.
