Community Care Live is looking for an experienced Adult's Social Worker to join their Community Hospital Team. This role focuses on individuals aged 18+ preparing for discharge, ensuring safe and person-centred transitions by assessing needs and arranging necessary support.
The ideal candidate must have a Social Work degree and at least two years of post-qualified experience, preferably in a hospital or assessment team. A UK driving license and vehicle access are essential. In addition, the position offers up to £35.00 per hour alongside a hybrid working scheme and supportive management.
