We are looking for an Adult's Social Worker to join a Forensic Community Outreach Team. Do not apply for this position if you do not have a Social Work Qualification with a minimum of 2 years post qualified experience.

About the team:

  • This service works with service users involved in the justice system that are dealing with mental health concerns who have been or are due to be discharged from hospital.
  • Carrying out discharge processes, safeguarding inquiries, extensive risk assessment and care plans is key in this team.
  • Having a strength-based practice and promoting positive change in people's lives is key to the success of this role and team.
  • This is an NHS MDT team and so functions and processes will follow this way of working.

About you:

  • A degree within Social Work (Degree/DipSW/CQSW) with a minimum of 2 years experience is essential in order to be considered for this role.
  • Experience in a mental health, hospital or forensic setting post qualification is essential in this position.
  • Having knowledge and experience in criminal justice, discharge processes or CoP work lends very well to the success of this role.
