Hoop Social Work is delighted to be working with Cardiff Council to recruit an experienced Community Mental Health Social Worker to join their well-established Community Mental Health Team. This is an excellent opportunity for an experienced practitioner who enjoys working in a fast-paced, person-centred environment, supporting adults with complex mental health needs to maximise their independence and achieve positive outcomes.
The Role
- Complete strengths-based assessments and care planning.
- Assess, commission and review care and support packages.
- Work within current legislation, policy and statutory guidance.
- Undertake planned interventions to promote independence and wellbeing.
- Safeguard adults who may be at risk or vulnerable.
- Work collaboratively with health professionals and partner agencies to deliver effective multi-agency support.
- Empower individuals to have choice, control and a strong voice in decisions affecting their lives.
- Contribute to maintaining high-quality services across the city.
To apply, you'll need:
- A recognised Social Work qualification.
- Registration with Social Care Wales or Social Work England.
- At least 6 months' UK experience working as a registered Social Worker.
- An Enhanced DBS on the Update Service (or be willing to obtain one).
- Three years of satisfactory references.
- A full UK driving licence and access to a vehicle.
