Our client is looking for an experienced Adult Social Worker to join their Older People's team, with bases across South, North and East Caerphilly (Ystrad Mynach and Rhymney). This is a well-established statutory role delivering assessment and care management services to adults requiring care and support to maximise their independence and dignity.

About the Role

The post holder will carry out holistic assessments of need and risk, develop and review care and support plans, and work in partnership with health colleagues — including Community Nurses, Community Psychiatric Nurses and the LHB Complex Care Team — to ensure integrated service delivery. The role involves direct work with individuals, families and carers, contributing to multi-agency decision-making forums and managing a varied caseload with supervision and support. There may also be occasional opportunities to deputise for the Senior Social Worker or Team Manager, and to mentor students or newly appointed staff. The role is predominantly community-based, with some lone working in clients' homes and community settings. A full UK driving licence and access to a vehicle insured for business use is essential.

Working Pattern

An average of 3 days in the office and 2 days remote, with flexibility required.

What We’re Looking For

  • Qualified Social Worker — Diploma or Degree in Social Work
  • Registered with Social Care Wales (or equivalent UK regulator)
  • Experience in Adult assessment and care management
  • Knowledge of the Social Services and Well-being Act (Wales), Mental Capacity Act, MHA and DoLS
  • Strong communication, organisation and multi-agency working skills
  • Welsh language skills desirable
  • Enhanced DBS registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving licence with access to a car
