Adults Social Worker needed for our Adult Social Care service in Bracknell paying up to £38 per hour.
About the Role:
Working within an integrated service, you will play a vital role in ensuring residents receive the right support at the right time. The service works closely with health partners and wider agencies to keep individuals safe and well in their communities, prevent unnecessary hospital admissions, and support timely and safe discharges when hospital care is required.
Key Responsibilities:
- Lead safeguarding enquiries, applying sound professional judgement to protect adults, manage risk, and meet statutory responsibilities.
- Manage a complex and varied caseload, prioritising effectively while maintaining high standards of practice.
- Work within the Adult Social Care Hub, collaborating with multidisciplinary teams to deliver timely and proportionate interventions.
- Build strong relationships with individuals, families, and community networks to promote independence and wellbeing.
- Apply a strengths-based approach to reduce reliance on long-term services and improve outcomes.
Essential Requirements:
- Qualified Social Worker with relevant professional registration.
- Experience within Adult Social Care, ideally in a local authority setting.
- Strong safeguarding knowledge and decision-making skills.
